It is the hairline — where it sits, how it curves, and how carefully its edge has been made imperfect. A hairline is the one part of the result you cannot revise later, so at HairGrow it is drawn before anything else is discussed, and you approve every line before a date is booked.

A hairline is a decision about your face at 55, not at 30

Natural hairlines recede. A design placed where a twenty-five-year-old’s hairline sits will look convincing for a few years and increasingly wrong after that, because the native hair behind it keeps thinning while the transplanted line stays exactly where it was placed. We therefore design to the face you will have in two decades: a line that reads as mature, not juvenile, and that leaves donor capacity in reserve for a second session if your loss pattern progresses.

In practice this means the frontal line usually sits a finger’s width or more above the highest forehead crease, with the temporal points set back rather than squared off. It looks conservative on the drawing. It looks correct in photographs for the rest of your life.

Three measurements that do most of the work

1. Angle

Hair does not leave the scalp vertically. At the frontal line it exits at roughly 15–25 degrees, lying almost flat against the forehead, and the angle steepens as you move back over the crown. Grafts placed at the wrong angle produce hair that stands up no matter how it is styled — the classic giveaway of older transplant work. Every recipient site at the frontal line is opened by hand, at the angle of the hair that used to grow there.

2. Density gradient

Real hairlines are not uniformly dense. They start sparse at the very edge, thicken over the first centimetre, and reach full density behind that. Loading the front edge with maximum density is the fastest way to make a transplant obvious, and it wastes grafts that would do more work further back. We build a gradient: single-hair follicular units at the edge, then doubles, then triples and quadruples behind.

3. Irregularity

No natural hairline is a smooth arc. It has small clusters, gaps and a soft, broken transition zone. A deliberately irregular edge — sometimes called a “sawtooth” — is what makes the result survive close inspection under bad lighting. It is designed, not left to chance.

What happens in your consultation

You send two photographs: one straight on with the hairline dry and pushed back, one from above showing the crown. From those we assess the Norwood stage, estimate donor capacity from the density of the safe zone, and produce a graft number that is a real estimate rather than a sales figure.

At the design appointment — video or in-clinic — the line is drawn on your scalp and photographed from four angles. You see it before you commit. If you want it lower, we will tell you honestly whether that is sustainable given your donor area, and what it would cost you in ten years.

A transplant should be invisible as a transplant. Everything in the design stage exists to serve that one test.

Questions worth asking any clinic

Who draws the hairline — the surgeon, or a consultant?

Who opens the recipient sites at the frontal line?

What is the planned density at the edge versus behind it?

How much donor capacity is being left in reserve?

Will I see and approve the design before scheduling?

If the answers are vague, keep looking. If you would like ours, send two photos and you will have a documented plan and a graft estimate within 24 hours — free, with no obligation.

A transplanted hairline is a permanent decision.
