Clinics market Sapphire FUE and DHI as if they were different operations. They are not. Both extract individual follicular units from the donor zone at the back and sides of the head, and both implant them into the thinning area. What differs is a single step in the middle — how the recipient site is created — and that one difference has real consequences for healing, density and whether you have to shave your head.
What each name actually refers to
Sapphire FUE
Follicular Unit Extraction with sapphire blades. The surgeon first opens the recipient channels with a sapphire-tipped blade, then places the grafts into those channels. Because the channels are made in advance, the surgeon can see the whole design at once and adjust angle and density across the entire area before a single graft goes in.
The sapphire tip matters: it makes a narrower, smoother channel than a steel blade, which means less tissue trauma, tighter graft spacing and faster crust clearance.
DHI (Direct Hair Implantation)
The channel and the placement happen in one motion, using a Choi implanter pen. The graft is loaded into the pen and pushed into the scalp directly. No pre-made channels, less time out of the body for the graft, and — because the surrounding hair is not disturbed — no need to shave the recipient area.

How we actually choose
Three factors decide it, and none of them is fashion.
- How much area needs covering. Above roughly 3,500 grafts, DHI’s per-graft time makes a single session impractical. Large restorations go to Sapphire FUE.
- Whether there is native hair in the way. Adding density between surviving hairs is exactly what the Choi pen is good at — it slots grafts in without cutting channels through healthy follicles. This is why DHI dominates hairline refinement and female restoration.
- Whether you can shave. If shaving is impossible for work or personal reasons, the no-shave DHI route exists and costs more for a reason: it is slower.
Frequently the answer is both — Sapphire FUE across the mid-scalp and crown, DHI for the frontal line. A technique should be chosen after the design, not sold before it.
What does not change between them
Graft survival is decided by handling, not by the tool: time out of the body, holding solution temperature, how gently the technician grips the graft, and the surgeon’s angle discipline. A clinic with excellent handling will beat a clinic with the fancier acronym every time.
